Government Crackdown on Nursing Home Abuse in Nassau and Suffolk Counties

Recent government enforcement actions have targeted several nursing homes in Nassau and Suffolk counties due to issues such as financial fraud, resident neglect, and mistreatment. Notable cases include:


Fulton Commons Care Center, Inc. (East Meadow, Nassau County):

• December 2022: The New York Attorney General filed a lawsuit against Fulton Commons, its owners, related parties, and former administrator, alleging a fraudulent scheme that led to insufficient staffing and significant resident neglect, mistreatment, and abuse.

• March 2024: A settlement was reached requiring Fulton Commons’ owners and operator to pay up to $8.6 million and implement significant reforms to address financial fraud and resident mistreatment.


Cold Spring Hills Center for Nursing and Rehabilitation (Woodbury, Nassau County):

• December 2022: The Attorney General filed a lawsuit against Cold Spring Hills, its related businesses, owners, and senior managers, alleging years of financial fraud and self-dealing that led to severe understaffing and resident neglect.

• December 2024: The Attorney General intervened to prevent the facility from shutting down and evacuating over 300 residents without proper notice, calling the attempted closure illegal.


Centers Health Care Facilities (Various Locations):

• June 2023: A lawsuit was filed against the owners, operators, and landlords of four nursing homes managed by Centers Health Care, alleging the illegal diversion of over $83 million in taxpayer funds, resulting in significant resident neglect and harm.

• November 2024: A settlement was reached requiring the involved parties to pay $45 million and implement major reforms to address years of resident mistreatment and neglect.


Medford Multicare Center for Living, Inc. (Medford, Suffolk County):

• February 2014: Nine employees, including the facility’s top administrator, were arrested and later indicted in connection with the death of a 72-year-old resident and subsequent attempts to cover up the circumstances surrounding the death.

As of the latest available information from the Centers for Medicare & Medicaid Services (CMS), the Special Focus Facility (SFF) Program identifies nursing homes with a history of serious quality issues. Facilities in this program are subject to increased oversight to stimulate improvements in their quality of care.


Special Focus Facility (SFF) Program Overview:

• Purpose: To monitor nursing homes that have consistently underperformed in health inspections and have a record of serious quality issues.

• Process: Facilities designated as SFFs receive more frequent inspections and are subject to progressive enforcement actions if improvements are not made.
